Rectangular wooden box covered with black leather effect paper. It has a press stud release catch and is hinged along the shorter length. The inside of the lid section is lined with white silk and the other with royal blue velvet. This is pierced to take the pin of the medal and has a circular indentation to fit the medal itself.

The medal consists of two parts joined by a small metal ring. The upper part is of two silver coloured pieces shaped and joined together with a royal blue piece of ribbon. The top shape has a pin joined at right angles. The lower part is a circular medal of silver coloured alloy. Both sides have a decoration of scrolls and leaves in a circle. The front has a wreath and the words 'FOR REGULAR ATTENDANCE' embossed on it, while the reverse has the name 'E. DAVIS' engraved on it.
