This print depicts the keystone (central stone of the arch) of a new gate in the walls of Rome. Pope Pius IV commisioned this new gate called the Porta Pia from Michelangelo in 1561 It may have been Michelangelo's assistant, Jacomo del Duca who actually carved the keystone.
